Individual medical insurance premium reimbursement
After 90 cumulative days worked on contract assignment, you will be eligible for
reimbursement of 50 percent of your own individual medical insurance premium*, up
to $100 per month. Eligibility is effective the first day of the month following
90 days worked on contract assignment, and is contingent upon a minimum of 100 hours
worked each month thereafter.
*COBRA and group plans administered by an employer are not eligible for reimbursement.
401(k) plan
After you have worked 1,000 hours worked on assignment with Pharmacy Placement Professionals
in a 12 month period, you will be eligible to enroll in our 401(k) retirement savings
plan, effective the first day of the following calendar quarter. You may contribute
1 to 100 percent of your salary (up to $15,000 in 2006) with a choice of several
fund options. Individual contribution limits may vary based on your compensation.
The company may match a percentage of your contribution at the end of each calendar
year, with full vesting for any company match after two years.
